Seguimiento farmacoterapéutico y alianza terapéutica en pacientes VIH positivo con perfil de fallo inmunológico y virológico

Resumen Introducción: El virus de inmunodeficiencia humana fue introducido en la población al este de África antes de 1959, la epidemia se evidenció en 1981. Más de 27 antirretrovirales aprobados por la FDA se utilizan para conformar el tratamiento antirretroviral altamente activo (TARAA), pero la aparición de fallas virológicas e inmunológicas se siguen presentando en personas con VIH/SIDA. En adición, efectos secundarios inherentes al TARAA contribuyen al desarrollo del síndrome metabólico (SM), diabetes y riesgo de enfermedad cardiovascular. La atención farmacéutica facilita el logro de los resultados esperados mediante las intervenciones farmacéuticas que tratan de modificar alguna característica del tratamiento, del paciente o de las condiciones que lo envuelve para resolver o prevenir resultados negativos asociados a la medicación (RNM) y problemas relacionados con los medicamentos (PRM) con el fin de preservar o mejorar los resultados positivos alcanzados. Objetivo: Analizar el impacto de la alianza terapéutica y del seguimiento farmacoterapéutico en los marcadores moleculares, bioquímicos y clínicos, relacionados a la falla inmunológica y/o virológica, así como al desarrollo de síndrome metabólico y riesgo cardiovascular (RCV) en pacientes que viven con VIH/SIDA (PVV). Material y métodos: El estudio se llevó en la Clínica de Retrovirus, HGR-MF No.1, IMSS, Morelos, donde se realizó el análisis de falla inmunológica y virológica, se calculó el RCV y se identificó a pacientes con síndrome metabólico, que fueron invitados a participar en un seguimiento fármaco terapéutico utilizando un diseño de estudio analítico, longitudinal y cuasi experimental. Resultados: La prevalencia de falla virológica fue de 4% y de falla inmunológica de 5.9% de un total de 460 PVV. El RCV fue de 2.3% en mujeres y de 4.2% en hombres. Un valor de RCV <7.5% fue considerando bajo y ≥7.5% alto, de este modo 388 pacientes fueron categorizados en RCV bajo y 72 en RCV alto. Con los criterios de ATP-III, 133 PVV (24.6%) tienen SM, de estos 82 pacientes aceptaron participar en un seguimiento farmacoterapéutico y alianza terapéutica, donde inicialmente se identificaron 102 PRM y 48 a los 12 meses. Estos PVV tuvieron un RCV basal de 6.3% que disminuyó a 5.9% a los 12 meses. De los 82 PVV en seguimiento 56 se categorizaron en bajo RCV y 23 en alto.

Abstract Introduction: The human immunodeficiency virus was introduced in the population of East Africa before 1959, the epidemy was evident in 1981. Starting from the structure, life cycle and interactions with the host, antiretroviral drugs (ARV) have been generated against HIV; more than 27 FDA approved ARV are used to shape the highly active antiretroviral therapy (HAART), but the appearance of virologic failure keep appearing in people living with HIV/AIDS (PVV). Additional to this, side effects inherent to HAART contribute to the development of the metabolic syndrome (MS), diabetes, and cardiovascular disease risk (CVR). Pharmaceutical care facilitates the achievement of the expected results through pharmaceutical interventions that try to modify some feature of the treatment, the patient or the conditions that surround it to resolve or prevent negative results associated with medication (NRM) and problems related to medication (PRM) with the to preserve or improve the positive results achieved. Objective: To analyze the impact of therapeutic alliance and pharmacotherapy follow-up on molecular, biochemical and clinical markers, related to immunological and/or virologic failure, as well as the development of MS and CVR in PVV. Methods: The study was conducted at the Retrovirus Clinic, HGR-MF No.1, IMSS, Morelos, where the analysis of immunological and virologic failure was carried out, the CVR was determined and patients with MS were identified and then invited to participate in a pharmacotherapy follow-up using an analytical, longitudinal and quasi-experimental study design. Results: The prevalence of virologic failure was 4% and immunological failure of 5.9% of a total of 460 PVV. The CVR was 2.3% in women and 4.2% in men. A CVR <7.5% was considered as low and ≥7.5% as high, in this way 388 patients were categorized in low CVR and 72 in high CVR. According to the criteria of ATP-III, 133 patients (24.6%) had MS, from these, 82 patients accepted to participate in a pharmacotherapy follow-up and therapeutic alliance, where initially 102 PRM were identified, and 42 after 12 months. They had basal CVR of 6.3% that decreased to 5.9% after 12 months. Of the 82 PVV in follow-up, 56 were categorized with low CVR and 23 with high CVR.
