Efecto de polímeros hidrofílicos en los perfiles de disolución de un sistema cocristalino de praziquantel con ácido L-málico

La esquistosomiasis es una de las enfermedades parasitarias más importantes en el mundo debido a su gran impacto, pues cerca de 200,000 personas al año mueren a causa de ella. Praziquantel (RSPZQ) es el ingrediente farmacéutico activo (IFA) de elección para el tratamiento de esta enfermedad; sin embargo, presenta baja solubilidad acuosa (0.4 mg/mL) comprometiendo su eficacia terapéutica. Además, es comercializado como racemato, aun cuando se ha demostrado que es solo el enantiómero “R” con actividad antiparasitaria, por lo que se administra en altas dosis (600 mg). Estudios previos realizados dentro del grupo de investigación permitieron la resolución quiral de RS-PZQ mediante la formación de cocristales diastereoméricos con ácido L-Málico obteniendo R-PZQ hemihidratado. Asimismo, al evaluar la velocidad de disolución intrínseca (VDI) se observó una mejora en la solubilidad y disolución de PZQ proveniente del cocristal R-PZQ:L-Mál con respecto a RS-PZQ y al hemihidrato de R-PZQ. Sin embargo el estado sobresaturado que se genera a partir del cocristal RPZQ:L-Mál se ve comprometido a los primeros minutos de la prueba de disolución de polvos en medio acuoso (HCl, pH 1.2), dada la precipitación R-PZQ que en consecuencia afectará la absorción del IFA. Por tal motivo, el objetivo del presente estudio fue evaluar el efecto de polímeros inhibidores de la precipitación (PIP) sobre los perfiles de disolución del cocristal R-PZQ:L-Mál (estequiometria 1:1) tanto en condiciones saturadas (disolución de polvos) como no saturadas (aparato I USP) con la finalidad de observar si se puede generar un aumento en la concentración disuelta de PZQ proveniente del cocristal R-PZQ:L-Mál. Para fines de comparación se incluyeron en estos estudios el RS-PZQ y el hemihidrato de R-PZQ. Se observó por medio de la técnica de disolución de polvos en condiciones saturadas que metilcelulosa (MC) e hidroxipropilcelulosa (HPC) predisueltos al 0.5 % peso/volumen ejercen un efecto positivo al aumentar y prolongar la sobresaturación generada por el cocristal (1.9 y 1.5 veces más para MC y HPC, respetivamente, en comparación con R-PZQ sin polímero). Las pruebas de disolución en condiciones no saturadas, mostraron que los cocristales diastereoméricos RS-PZQ:L-Mál y el cocristal R-PZQ:L-Mál aumentan significativamente la concentración disuelta de PZQ con respecto a los fármacos puros; sin embargo, al adicionar los polímeros, estos no ejercieron un efecto sobre los perfiles de disolución de dichos cocristales, pero sí sobre las fases de los fármacos puros. Se encontró que al formular el hemihidrato de R-PZQ con HPC por molienda mecánica en un molino de perlas (10 min, 10 Hz) a una proporción de 1:0.3 peso/peso (fármaco:polímero) hay un aumento significativo en la concentración de R-PZQ que excede al efecto observado a partir del cocristal (16 veces mayor con respecto a RS-PZQ). Asimismo, la formulación por molienda del hemihidrato de R-PZQ con HPC 0.3 p/p exhibió un perfil de disolución muy parecido al obtenido por la formulación comercial de RS-PZQ (CITICID®), lo que sugiere desarrollar una formulación sólida basada en este componente para reducir la dosis comercial y con esto disminuir los efectos adversos asociados a RS-PZQ.

Schistosomiasis is one of the most important parasitic diseases in the world, since almost 200,000 people die from it. Praziquantel (RS-PZQ) is the active pharmaceutical ingredient (API) of choice for treatment of this disease; however, it has low aqueous solubility (0.4 mg/mL) that compromises the therapeutic efficacy. In addition, it is marketed as the racemate, even though it has been shown that only the "R" enantiomer has antiparasitic activity (eutomer); because of this, PZQ must be administered in high doses (600 mg). Previous studies of our research group enabled the chiral resolution of RS-PZQ through the formation of diastereomeric cocrystals with L-Malic acid to give R-PZQ hemihydrate. Evaluation of the intrinsic dissolution rate (IDR) showed an improvement for cocrystal R-PZQ:L-Mal with respect to RS-PZQ and the hemihydrate of R-PZQ. However, the supersaturation state generated from co-crystal R-PZQ:L-Mal is lost already in the initial phase of the powder dissolution test, in aqueous medium (HCl, pH 1.2), due to the precipitation R-PZQ, which affects the absorption process. Because of this the objective of this study was to evaluate the effect of polymers as precipitation inhibitors (PPIs) on the dissolution profiles of co-crystal RPZQ:L-Mal (1:1 stoichiometry) in both saturated (shake flask) and unsaturated conditions (apparatus I USP) to observe if an increase in the dissolved concentration of R-PZQ can be achieved. For comparison, the profiles of RS-PZQ and R-PZQ hemihydrate were also included in the study. The powder dissolution tests in saturated conditions showed that methylcellulose (MC) and hydroxypropylcellulose (HPC) which were pre-dissolved at 0.5% weight/volume, increased and prolongated the supersaturation state generated by the co-crystal (1.9 and 1.5-fold for MC and HPC, respectively, in comparison to R-PZQ without polymer). The dissolution tests under unsaturated conditions, showed that the diastereomeric co-crystals RS-PZQ:L-Mál and co-crystal R-PZQ:L-Mál significantly increase the percentage dissolved of PZQ with respect to the pure drugs; however, when polymers were added to cocrystals, an effect on the dissolution profiles was not observed. Nevertheless, the dissolution profiles of the pure phases of PZQ increased significantly in the presence of polymers. It was found that R-PZQ hemihydrate formulated with HPC by mechanical grinding in a bead mill (10 min, 10 Hz) at a ratio of 1:0.3 w/w (drug: polymer) exhibits a significant increase in the concentration of R-PZQ that exceeds the effect observed from the co-crystal (16 times greater in contrast to RS-PZQ). Likewise, it was found that the grinding formulation of R-PZQ hemihydrate with HPC 0.3 w/w exhibited a dissolution profile very similar to that obtained by the commercial formulation of RS-PZQ (CITICID®). This suggests developing a solid formulation based on these components to reduce the commercial dose and with to decrease the adverse effects associated with RS-PZQ.
