Identificación de prescripciones potencialmente inapropiadas en pacientes de 65 años o más hospitalizados en el ISSSTE de ciudad Valles, San Luis Potosí

La prescripción potencialmente inapropiada de medicamentos (PPI) en la población geriátrica es considerada un problema de salud, ya que está asociada a un aumento en el riesgo de presentar eventos adversos, morbilidad, mortalidad y una mayor utilización de los recursos sanitarios. En general, un medicamento se considera apropiado cuando presenta evidencia que apoya su uso en una indicación determinada y si es bien tolerado en la mayoría de los pacientes. Para cumplir con este criterio y minimizar los problemas que surgen como consecuencia del uso de medicamentos inapropiados en los pacientes geriátricos, se han desarrollado criterios de consenso para el uso seguro de los medicamentos en esta población, tales como los criterios de Beers o los Screening Tool of Older People´s Potencially Inappropriate Prescription (STOPP). Por tanto, el objetivo del presente estudio fue identificar la frecuencia de las PPI, así como los factores de riesgo asociados a ellos, utilizando los criterios Beers y STOPP, en pacientes de 65 años o más hospitalizados en el ISSSTE de ciudad Valles, San Luis Potosí. A través de la revisión de los expedientes clínicos de los pacientes hospitalizados, se identificaron las PPI con los criterios de Beers y STOPP/START, así como los posibles factores de riesgo que estuvieron relacionados a la presencia de una PPI en la población. Los datos obtenidos fueron analizados por un análisis bivariado y multivariado. Los resultados obtenidos mostraron que el 43.5% de la población ingresada presentaron al menos una PPI y una edad media de 78 años, con un promedio de 8 fármacos usados por paciente; el análisis de los datos nos sugirió que la presencia de polifarmacia y el uso de metformina son estadísticamente significativos a la presencia de una PPI con valores de p < 0.05 (OR =10.553) y p 0.009 (OR=3.227) respectivamente. Por tanto, podemos concluir que es muy frecuente la presencia de PPI en la población geriátrica hospitalizada en el ISSSTE de Cd. Valles, en la cual la presencia de la polifarmacia juega un rol importante con respecto a la prescripción inapropiada.

The potentially inappropriate prescription (PIP) in geriatric population is considered a public health problem, since this is associated with an increase in the risk of adverse events, morbidity, mortality, and a greater use of health resources. Generally, a drug is considered appropriate when it presents evidence that supports their use in a particular indication and if it is well tolerated in most patients. To meet this criterion, and minimize the problems that arise as a consequence of the use of inappropriate medication in geriatric patients, they have been developed criteria of consensus for the safe use of the drugs in this population, such as the Beers criteria or the Screening Tool of Older People´s access Inappropriate Prescription (STOPP). Therefore, the aim of the present study was to identify the frequency of the PIP as well as associated risk factors with them, using the Beers and STOPP criteria in patients 65 years or older hospitalized in ISSSTE of Valles city, San Luis Potosí. Through a review of the clinical records of hospitalized patients, the PPI with the Beers and STOPP/START criteria was identified, as well as possible risk factors that were related to the presence of a PIP in the population. The data obtained were analyzed by bivariate and multivariate analysis. The results showed that 43.5% of the population entered presented at least one PIP, an average age was 78 years, with a mean of 8 drugs used by patient. Data analysis suggested that the presence of polypharmacy and the use of metformin were statistically significant to the presence of a PPI with p values < 0.05 (OR = 10.553) and p 0.009 (OR = 3.227) respectively. Therefore, we can conclude that the presence of PPI is very frequent in the geriatric population hospitalized in ISSSTE of Valles City, in which the presence of polypharmacy plays an important role with regard to the inappropriate prescribing.
