Asociación entre presión aórtica central, sus derivados y malnutrición en pacientes con enfermedad renal crónica en hemodiálisis

RESUMEN Existe evidencia de la relación entre rigidez arterial y malnutrición en el paciente con hemodiálisis. La presencia de malnutrición independiente de la inflamación puede contribuir a la disfunción vascular. El objetivo fue determinar la asociación entre el estado de malnutrición-inflamación con la rigidez arterial, ya que la detección oportuna de estas complicaciones podría optimizar su pronóstico y tratamiento. Estudio transversal prospectivo y analítico. Pacientes con Enfermedad Renal Crónica en Etapa 5 en hemodiálisis por ≥ 3 meses, de cualquier etiología (febreroagosto 2019). Se realizó toma de Presión Aórtica Central, Velocidad de Onda de Pulso e Índice de Aumentación, por método oscilométrico no invasivo. Se obtuvieron albúmina y transferrina sérica el mismo día, se calculó el score de Malnutrición- Inflamación (MIS) de cada paciente y el consumo de alimentos por recordatorio de 24 horas. Con una muestra de 25 pacientes; 16 obtuvieron un score MIS de riesgo bajo (≤ 7 puntos) y 9 de riesgo alto (≥ 8 puntos). Se encontró una correlación inversa entre el score MIS y la PAC (p=0.006), sugiriendo un mayor cambio en el porcentaje de la PAC si se comparan los valores antes y después de la hemodiálisis, con relación al riesgo bajo MIS. El IAu, marcador de rigidez arterial, mostró poco cambio en los pacientes con riesgo alto MIS (p=0.003), sugiriendo aumento de rigidez arterial. En este studio se observa que en los pacientes con ERC en hemodiálisis existe un mayor estado de malnutrición-inflamación el cual se asocia con un aumento de la rigidez arterial. Se sugiere el uso del score MIS en la clínica, al prescribir tratamiento antihipertensivo y antiinflamatorio en pacientes con ERC en hemodiálisis.

ABSTRACT There is evidence of a relationship between arterial stiffness and malnutrition in patients undergoing hemodialysis. The presence of malnutrition regardless of inflammation may contribute to vascular dysfunction. The aim was to study the association between the malnutrition-inflammatory status and arterial stiffness, which could help to detect potential complications timely to optimize their treatment. Prospective and analytical cross-sectional study. Patients in Stage 5 Chronic Kidney Disease under hemodialysis for ≥ 3 months, of any etiology (February- August 2019). Central Aortic Pressure, Pulse Wave Velocity and Augmentation Index were taken by a non-invasive oscillometric method. Albumin and serum transferrin were obtained on the same day, the Malnutrition-Inflammation Score (MIS) was calculated for each patient and food consumption by 24-hour reminder. A sample of 25 patients was obtained; 16 had obtained a low risk MIS score (≤ 7 points) and 9 had a high risk one (≥ 8 points). We found an inverse correlation between a highrisk MIS score and CAP (p=0.006), suggesting a greater change in the percentage of CAP before and after hemodialysis in relation to low risk MIS. The IAu arterial stiffness marker showed little change in percentage in patients with high risk MIS (p=0.003), suggesting increased arterial stiffness. In this study it is found that hemodialysis patients with CKD there is an increase in the malnutrition-inflammation status which is associated with increased arterial stiffness. The use of the MIS score in clinical practice is suggested, when prescribing antihypertensive and anti-inflammatory therapy in patients with CKD in hemodialysis.
